Participants complete questionnaires at multiple time points: before treatment, then at 3, 12, 18 and 24 months after consent and then annually.

Case Report Forms (CRFs)

Participants will also consent to the collection of clinical data by staff at sites using Case Report Forms (CRFs) at the start of treatment (baseline), then at 6 and 12 months after and then annually.
