Online Questionnaires

HORIZONS Study Follow-Up Questionnaires (3 months onwards) are available for participants to complete online.

• The online questionnaires are easy to use and are laid out like the paper version
• You can access it on your computer or tablet device at any time
• You can create your own log in details
• It will allow you to take breaks and return to a ‘saved’ version
• All answers you provide will be kept strictly confidential and saved on our secure servers

If you would like to complete your follow up questionaires online please contact HORIZONS@soton.ac.uk with 'Online Questionnaire Completion' as the email subject. It would be helpful if you could include your participant number in this email. We will check your HORIZONS details and send you a link for the next questionnaire due.

If you prefer paper questionnaires, there is no need to do anything, we will post them to you with a pre-paid return envelope for you to return it to us.
